I got the long form in November. Many of the questions I have no idea how to answer since I'm not privy to private information of relatives, landlords etc. I had it less than a week when the calls started coming of it being late, they need it now etc etc. I finally decided that I would answer only questions I was at that time 100% sure about which was basically about 5. I got another call about a week after it was sent in which they literally threatened me with jail if it was not completed to their satisfaction.
